Licensing

• 5 copies of the application form completed in CAPITAL LETTERS and clear (including the original form)
• 2 copies of Form A (Business Registration) and Form B and Form D (Registration of Business Changes) according to the Business Registration Ordinance 1965 (SSM Address is the same as the Business Address) or Memorandum and Articles of Association for the Company Form 24 (Return of Allotment of Shares) and Form 49 (Return Giving Particulars in Register of Directors, Managers and Secretaries and Changes of Particulars) in accordance with the Company Registration Act 1965 which has been certified by the Company Secretary.
• 2 copies of the Rental Agreement (if rented) which has been stamped or the permission of the owner of the building (if not the building itself) which has been certified by the Commissioner of Oath / Commissioner of Oath.
• 2 copies of the grant (if an old building / private land) or a copy of the Lease Purchase Agreement
• 3 copies of Certificate of Eligibility to Occupy the Building ( CF )
• 2 copies of the Grant or 'Sales and Purchase Agreement'
• 3 copies of assessment tax receipts (already paid)
• 2 copies of land tax receipts
• 4 photos of the premises (4 different sides and 4 colored copies 1 front, 1 back, 2 inside)
• 5 copies of the location plan (not Google Maps)
• 2 copies of the floor plan
• 3 copies of the sketch showing the complete details of the advertisement
• 3 copies of the approval of the Language and Library Council
• 2 recent passport size photos of the applicant.
• 3 copies of the applicant's identity card
• 2 copies of doctor's examination letter including Typhine VI injection- For Food and Beverage Operators (Including applicants & Employees & IC)
• 2 copies of Food Handling course certificate- For each food and beverage operator only (Including applicants & Employees & IC)
• 1 copy of internal / external modification document
• 1 copy of fire extinguisher purchase receipt.

• This application form must be accompanied by the following documents:

  • 5 copies of the completed application form (including the original form)
  • 5 copies of Business License / Business Registration
  • 5 copies of sketches showing the complete details of the advertisement
  • 5 copies of the site plan or location plan according to scale 1:750 showing the position of the ad
  • 5 copies of Identification Card
  • 5 copies of the approval of the Language and Library Council

• This application form must be complete with the following documents:

  • Bahasa Malaysia must be used for all advertisements, whether alone or together with other languages.
  • If Bahasa Malaysia is used together with any other language in an advertisement, the words in Bahasa Malaysia shall be larger in size and shown more clearly in terms of visual priority and position than words in any other language and the use of Bahasa Malaysia Malaysia should be in the right way.
  • No person shall display, cause or permit to be displayed any advertisement which does not comply with paragraph E (1 and 2)
  • For a temporary display (Billboard) of Public Liability Insurance that has a cover value of at least RM50,000.00.
  • For temporary display applications (Billboard) include 3 copies of the advertisement sketch.
  • The Majlis reference number must be stated in the lower left corner of the advertisement

Banners and Bunting

1. Banners or 'bunting' should be taken to the Hulu Selangor Municipal Council Office to be stamped and signed by the Licensing Officer.
2. A copy of the sketch / visual of the banner or 'bunting' should be included with the application.
3. Maximum Size Allowed: i. Stretch Fabric (12 x 4 square feet) ii. Bunting (3 x 6 square feet)
4. The fee charged is as follows (per month):-

Terms of application:

1. Make sure there is MALAYSIAN LANGUAGE on the banner or 'bunting'.
2. If Bahasa Malaysia is used together with any other language in each banner, the words in Bahasa Malaysia should be larger and shown more clearly in terms of their position than the words in other languages and the use of Bahasa Malaysia should be in the correct way.
3. Claims for collateral must be made within SEVEN (7) DAYS after the permit EXPIRATION DATE by filling in the collateral form and submitting the original receipt, a copy of the identity card and carrying a banner/ 'bunting' (if necessary).
4. The security deposit is considered forfeited if the security deposit claim is made after SEVEN (7) DAYS from the permit expiry date or the Council finds that the advertisement has not been taken down after the approval period has expired.
5. The security deposit will not be exempted for the installation of banners/'bunting' that are exempted from the Council. For cases where the permit fee is exempt, applicants need to pay RM3.00 for each sticker.
6. The Council is not responsible for any loss, theft and damage to the banner or 'bunting'.
7. Every exception other than those mentioned above can only be granted with the written permission of the Chairman or Deputy Chairman of the Hulu Selangor Municipal Council.
8. The Council reserves the right to confiscate the 'bunting' / banner if it is hung in an inappropriate location.

Billboard Plus

This application form must be accompanied by the following documents:-

THE FOLLOWING ADDITIONAL CONDITIONS:-

1. Bahasa Malaysia must be used for all advertisements, whether alone or together with other languages.
2. If Bahasa Malaysia is used together with any other language in an advertisement, the words in Bahasa Malaysia shall be larger in size and shown more clearly in terms of visual priority and position than words in any other language and the use of Bahasa Malaysia Malaysia should be in the right way.
3. No person shall display, cause or permit to be displayed any advertisement that does not comply with paragraph E (1 and 2).
4. For a temporary display (Billboard) of Public Liability Insurance that has a cover value of at least RM50,000.00.
5. For temporary display applications (Billboard) include 3 copies of the advertisement sketch.
6. The Majlis reference number must be stated in the lower left corner of the advertisement.

• Memorandum and Articles of Association and Form 49 (statement giving details in the Register of Directors and Changes in Details) in accordance with the Companies Act 1965.

  or

• Form A (Business Registration) or Form B (Registration of Business Exchange) and Form D (Business Registration Declaration) according to the Business Registration Act 1965.
• A copy of the proposed Certificate of Occupancy (CFO) approval.
• If it involves a temporary structure required for entertainment purposes, then the following documents must be included:-

• • Structure plan in question:
  • Plan approval letter from the relevant authority; and
  • Certificate from a Structural Engineer certifying that the structure will be in good condition for the period during which the entertainment is to be held.

• If the application wants to sell food and drinks must get approval from the relevant authorities.
• 8 copies of floor plan and location.
• Letter of approval/confirmation from the owner of the land or building.
• 8 copies of identification card
• 2 passport size application photos.
• 1 envelope measuring 9x4 and 2 thirty cent stamps.
• 2 photos of the premises (1 photo taken from the outside and 1 photo taken from the inside)

• 4 copies of the completed composite form (including the original form)
• 2 copies of Business Registration or Company Memorandum
• 2 copies of the stamped Rental Agreement or permission from the Hulu Selangor Municipal Council
• 4 copies of the location plan of the premises involved
• 3 copies of the sketch of the details of the advertisement
• 3 copies of the applicant's identity card
• 2 recent ID card sized photos of the applicant
• 2 copies of the food handling course certificate (for each food and beverage operator)
• 2 copies of doctor's certificate / examination letter including TY2 injection (for each food and beverage operator)

• 3 copies of the identity card of the applicant / Husband and Wife
• 2 passport size photos of the applicant
• 3 copies of the location plan (hawking area, place, route specify the relevant route plan)
• 3 copies of business registration (if any)
• 3 copies of Vehicle Registration Card (Truck / Van / Car / Motorcycle)
• 3 copies of Driving License
• 2 photos of the vehicle (front and left/right side)
• 2 copies of food handling course certificate (for food and beverage vendors)
• 2 copies of doctor's examination letter including TY2 injection (for food and drink vendors)
• 2 copies of the applicant's insurance coverage

• All dogs 3 months and older must be licensed.
• This license will expire on December 31 of the license year.
• The dog license fee is RM10.00 per dog.
• Lost licenses will not be replaced. The owner must apply for a new license and is required to pay RM10.00 for each license/dog.
• The metal badge issued by the Hulu Selangor Municipal Council must always be hung around the dog's neck at all times. It is the owner's responsibility to provide a hanging strap for the metal badge.
• The dog house should always be clean of dog excrement.
• Dog owners are responsible for ensuring that their dogs do not bark incessantly, wander off the premises, bite or disturb members of the public in public places.
• Dogs must always be leashed and wear a muzzle when taken out of the place where they are normally kept/kept.
• Dogs cannot be kept in flats or buildings that exceed 2 floors.
• Only 2 dogs are allowed at any one time.
• Any dog found loose in a public place, diseased, kept without a license, causing a nuisance to others or kept in an unauthorized place, will be seized, arrested and destroyed.
• The owner is required to prepare and display a "DOG HERE" sign in a place easily visible to the public.
• Owners who fail to comply with the above conditions will be subject to a compound of not more than RM1,000.00 or a fine of RM2,000 or imprisonment for not more than one (1) year or both.
• Copy of Identification Card (1 Copy), Applicant's Photo (1 Piece) and Dog Vaccine Injection (1 Copy).

• A processing fee of RM5.00 is charged for each application.

  • This permit is not transferable.
  • This permit must be brought along when the applicant wants to renew the permit and displayed while the applicant is hawking for inspection by the Council Officer on duty.
  • Payment must be made every 6 months and must be renewed before the period expires. Failure to renew the permit will result in the permit being automatically revoked.
  • The rental fee is RM5.00 per unit.
  • A fine of RM10.00 will be charged for the loss or damage of this permit.
  • The Council reserves the right to withdraw this permit without giving any reason.
  • The Council will not be responsible for any loss of life or property of the hawkers.
  • Hawkers must comply with all Council regulations from time to time.
  • All individuals with permanent resident status are not allowed to apply for a night market permit.
  • All hawkers who sell food/beverages must obtain an injection from the District Health Department for a period of 3 years and must undergo a Food Handling Course.
  • The permit card must be brought with you every time you renew your permit.

• CONDITIONS:
  • The application for a permit to park table chairs is only allowed outside the food premises.
  • Must submit a copy of 'Public Liability Insurance' amounting to RM50,000.00 (for the application for a permit to place table chairs outside the food premises).
  • The permit period is 3 months and must be renewed after the period expires.

• REQUIRED DOCUMENTS:
  • Processing fee RM5.00
  • 2 copies of the completed application form.
  • 1 photo of the applicant
  • 1 copy of identification card.
  • 1 copy of health inspection report and food course certificate (for food operators/entrepreneurs)
  • 2 copies of the floor plan of the chair and table area.
  • 3 photos of the location of the premises. (2 copies)
  • 2 copies of the business license of the Hulu Selangor Municipal Council.

• The application for a parking permit is only allowed outside the food premises.
• The permit period is 3 months and must be renewed after expiry.
• Only premises that have a license from the Hulu Selangor Municipal Council are eligible to apply.

  • 1 photo of the applicant
  • 1 copy of identification card.
  • 3 copies of the floor plan where the goods are placed.
  • 3 photos of the location of the premises. (front, left and right angles) (2 copies)
  • 2 copies of the Hulu Selangor Municipal Council business license.
• Note: Application forms can only be found at the counter of the Licensing Department, Hulu Selangor Municipal Council Office,
  Kuala Kubu Bharu.
